Declaration of Good Character

Coaches and team staff are in a position of trust and are respected for their integrity, honesty, and commitment to a code of ethics. I acknowledge that to be a uOttawa Gee-Gees Varsity Team or Competitive Club, I must be of good character.

I further warrant that my answers to the below questions are true and complete:

• Have never been found to have breached (including for misconduct or incompetence) a rule of a provincial, territorial, national, or international coaching or sport governing body and its regulations
• Have never violated the WADA code, the Canadian Anti-Doping Program, or an equivalent program in another country
• Have never been convicted of a criminal offence

If the answer to any of the above three questions is “yes”, please provide details of the breach/violation/offence including the circumstances, date, sanction, and who we may contact for further details in an email to danika.smith@uottawa.ca.

By adhering to this Declaration of Good Character, I am answering “no” to the above questions and hereby certify that I am of good character and am qualified to be a uOttawa Gee-Gees Varsity Team or Competitive Club coach or team staff member. I further warrant that I know of no circumstances whatsoever that would call in question my fitness to be a uOttawa Gee-Gees Varsity Team or Competitive Club coach or team staff member. I declare that the information in this declaration is true and complete to the best of my knowledge.

I understand that a false statement or misrepresentation may disqualify me for a position as a uOttawa Gee-Gees varsity or competitive club coach or team staff member
